I received a lien notice, but I do not wish to participate in the Private Lien Resolution Program. What should I do?

Even if you do not join the Private Lien Resolution Program, you still have an obligation to re-pay your insurance company, if the terms of your insurance plan say so. You have that duty whether or not you’re part of the Private Lien Resolution Program. So if you do not take part in the program, you and your attorney will need to talk to the insurance company and address that lien notice.
